Project Summary

Cervical cancer is one of the most common cancers affecting women worldwide. In orderto reduce the threat of cervical cancer, the Health Promotion Administration of ChineseTaipei, through health units and medical institutions, has provided Pap smearexaminations for women over 30 years old since 1995, and provided HumanPapillomavirus (HPV) vaccination for 13-year-old girls since 2008 according to WHOrecommendations. So far, the incidence and mortality rate of cervical cancer have been reduced by about 70%, and the HPV vaccination rate has reached more than 90%.

Considering the good results achieved from the above policies, we propose to share our results with APEC members/economies in the 2025 conference. Through exchanging experiences with other members, we could help them promote these policies and create a positive impact on women's health. This will enable them to work together towards the WHO's 90-70-90 goal.

Relevance

Region
Cervical cancer is one of the most common cancers affecting women worldwide, with more than 600,000 new cases and more than 340,000 deaths estimated in 2020. APEC economies accounted for an estimated 38 percent of new cases and 35 percent of deaths globally. The number of new cases and deaths is projected to increase from 570,000 to700,000 and from 311,000 to 400,000 per year, respectively, between 2018 and 2030.Cervical cancer and death from cervical cancer often occur at the age when women can contribute to society. Therefore, the occurrence of cervical cancer will have a great devastating impact on families, economies, women's rights, and social welfare.
Cervical cancer is a preventable disease through vaccination, screening, early detection, and early treatment. By implementing interventions across the prevention and control continuum, the burden of the disease can be reduced, and women can live a healthy and prosperous life. Chinese Taipei has provided annual cervical cancer screenings with Pap tests for women over 30 years old since 1995 through health units and medical institutions. In addition, in accordance with WHO recommendations, HPV vaccination was provided to 13-year-old girls in 2008. Through the upcoming conference, participants can work together to achieve the WHO’s 90-70-90 goal of eliminating cervical cancer by2030.

Eligibility and Fund Priorities

This project is eligible for the IERG (Innovative Development, Economic Reform and Growth) Sub-Fund based on the project intends to achieve the 2014“Healthy Asia-Pacific2020” initiative: Strive to promote the basic immunization coverage for children and encourage the utilization of new and underused vaccines; further disseminate knowledge on sexual and reproductive health.

Chinese Taipei has integrated its healthcare and education systems and actively promoted HPV vaccination services for junior high school girls through local health bureaus while disseminating cervical cancer health education messages. By establishing a follow-up tracking system between schools and health bureaus, adverse events related to vaccination can be monitored.

In terms of screening, Chinese Taipei utilizes information systems to proactively remind eligible women to undergo cervical screenings at medical institutions and provides mobile cervical screening services in communities. This improves the accessibility of cervical screening services. Additionally, healthcare professionals and health units educate the public about prevention, screening, and treatment, thereby enhancing the doctor-patient relationship. Abnormal screening cases are tracked and managed. The integration of these resources provides an opportunity for APEC economies to learn effective collaboration and increase the coverage rates of HPV vaccination and cervical cancer screening.

Capacity Building
(1) This project will enable member economies, particularly developing economies, to obtain, share, strengthen, maintain, and develop knowledge, abilities, skills, and technical know-how in order to improve their policies and regulations as well as relevant structures and processes.
(2) Promoting free HPV vaccine services, cooperating with educational institutions and other government agencies, delivering in-school vaccinations, and a system for reporting and monitoring adverse reactions has been established. Chinese Taipei closely monitors international policy developments, which will help increase awareness of HPV vaccines, increase HPV vaccination rates, and avoid the economic burden of cervical cancer.

(3) To increase awareness of cervical cancer prevention and treatment, enable early detection and early treatment, Chinese Taipei has been subsidizing women to receive annual cervical cancer screening services once a year for women over 30 years old. Through health units and medical institutions, the accessibility of Pap smear tests has been improved through services such as mobile screening vehicles and community screening stations, and follow-up management is provided for individuals who have tested positive to encourage them to receive treatment.

Objectives

The objective of this project is to strengthen member economies' capacity to prevent and control cervical cancer through a policy dialogue. Discussions will cover current strategies for government-funded HPV vaccination, cervical cancer screening (cervical cytology and HPV testing), and respond to challenges and obstacles in cervical cancer prevention and control.

Alignment

APEC
This project echoes the Status Report on Cervical Cancer Elimination in the APEC Regionin 2023 and aligns with the North Star—A 2030 Strategy and the WHO’s Global Strategyto Accelerate the Elimination of Cervical Cancer as a Public Health Problem, which waslaunched in November 2020 with targets set for 2030. It also contributes to theachievement of the APEC Roadmap to Promote Sustainable Economic Advancement forWomen through Cervical Cancer Prevention & Control, which was updated and re-endorsed by the HWG and 11th HLMHE in 2021.
This project aims to utilize information systems to understand the situation of vaccines and screenings. It will employ mobile clinics to provide screening services to rural and economically disadvantaged populations. The project also supports APEC Policy Partnership on Women in the Economy (PPWE) and is in direct alignment with the La Serena Roadmap for Women and Inclusive Growth (2019-2030). Women and girls in APEC member economies can live healthy and productive lives, with a substantially reduced burden of HPV‐related cervical cancer morbidity and mortality.

Forum
This project aligns with the mission statement of the "Health Working Group Strategic Plan 2021-2025," which aims to reduce the impact of health-related threats to the economy, trade and security of member economies, and to strengthen health systems to increase their efficiency, responsiveness and resilience to achieve and maintain universal health coverage (UHC). Through interdisciplinary and cross-sectoral collaboration, this initiative promotes HPV vaccination services, cervical cancer screening, and treatment. It aims to strengthen the health system for cervical cancer prevention, providing a safe, effective, high-quality, affordable, and sustainable primary healthcare for the entire population. In response to the "APEC Roadmap to Promote Sustainable Economic Advancement for Women through Cervical Cancer Prevention and Control 2021-2025".This project focuses on improving women health and promoting women labor participation.

Beneficiaries and Outputs

Output 1 - One-Day Conference
The one-day conference will be conducted fully in-person and will explore strategic actions for health information sharing in the public and private sectors, and is committed to facilitating APEC economies and the international community to exchange information and experience in eliminating cervical cancer.

I. The conference is scheduled to be held in Taipei City in March 2025. The conference will feature one session in the morning and two sessions in the afternoon, with the total conference duration estimated to be approximately7-8 hours. Following each session, there will be a discussion segment, during which experts will be invited to share their experiences and the latest scientific research findings related to cervical cancer prevention. The conference will conclude with a summary segment to recap key discussions and recommendations resulting from the Conference.

II. The conference will invite experts from APEC co-sponsoring economies who are dedicated to cervical cancer elimination intervention strategies to nominate moderators or speakers. Additionally, experts implementing relevant strategies from APEC member economies and international organizations will also be invited. They will introduce the latest developments in related fields, and share practical experiences, and discuss the prevention and treatment of cervical cancer. Experts will be recommended to attend the conference in person to have more engaged interactions.

III. In order to improve the cervical cancer prevention and control capabilities of APEC economies, the conference will aim to share (1) HPV vaccination strategies, data establishment, and monitoring of adverse reactions (2)cervical cancer screening strategies (Pap smear, HPV testing), difficulties, obstacles and responses to screening abnormality tracking, treatment, and cervical cancer prevention. etc., hoping to enable APEC members to work together toward the goal of eliminating cervical cancer. During each session,20-30 minutes will be allocated for discussion between the participants and speakers.

IV. Total participants: The conference aims to invite 80 participants, including5-6 speakers, who will be public health officers and researchers, health care workers, pharmaceutical professionals, and other relevant stakeholders from APEC member economies.

V. A questionnaire survey will be conducted upon completion of the conference to assess the satisfaction of participants and experts, as well as to collect feedback and suggestions.

Output 2 - Conference Report
Report will be produced after the completion of the conference, which will not be published by APEC but will be disseminated through the HPA's website and shared with all the participants and interested stakeholders.

The Report will consist of an introduction, table of contents, topic-based summaries of presentations, the results of the surveys, conclusions, and key findings and recommendations. In total, it will have at least 12 pages excluding annexes.

Outcome
Through this international conference, participants from APEC member economies will be able to share informationfrom their respective economies, raise awareness on the prevention and control of cervical cancer elimination,improve HPV vaccination, cervical cancer screening and treatment, and use information monitoring etc., thisknowledge acquisition and information dissemination will be measured through a questionnaire survey that will askparticipants what practices or recommendations they would apply in the workplace.

The project will support ongoing consultation, information exchange and collaboration among health sector experts across APEC economies on approaches to the management of non-communicable and chronic diseases. It is expected to effectively increase the HPV vaccination rate, cervical cancer screening rate and treatment rate in APEC member economies, and move towards the WHO's goal of eliminating cervical cancer.

Beneficiaries
APEC member economies, policymakers (such as government officials, and public health officials), service providers (such as health care providers, medical managers, and medical institutions), and relevant scholars (engaged in the elimination of cervical cancer) Cervical cancer-related researchers/scholars/analysts), schools and medical institutions, or students and the public who are interested in this topic (such as people who have received the HPV vaccine or cervical cancer screening and treatment), etc. During the conference, participants will share and discuss information and strategies related to the elimination of cervical cancer, and participate in discussions to enhance their understanding of this issue. We hope that through the sharing of professional information and practical experience, participants can understand the effective strategic actions taken by APEC member economies, learn from the experiences of other economies, improve the execution capabilities of action plans, and adopt best practices to formulate health promotion plans, increase HPV vaccination rates, cervical cancer screening rates, and treatment rates, thereby improving women’s health and economic growth in APEC member economies.

The direct beneficiaries and the APEC non-member participation process are as follows:

(1) Government officials (including health policymakers and public health officials in health promotion and medical affairs departments);
(2) Health care providers and medical managers;
(3) NGOs and international organizations such as WHO;
(4) Researchers in the fields of health services, digital health technologies, epidemiology, pediatrics, and obstetrics and gynecology to participate and contribute.
Through sharing experiences and knowledge about strategies to promote women's health and reduce cervical cancer mortality in this conference, we expect that the audience will directly benefit in three ways:
(1) Learn the successful experiences from other economies.
(2) Adopt the effective way to reduce cervical cancer mortality.
(3) Gain the knowledge to develop strategies to address the challenges faced.

Long-term or indirect beneficiaries: APEC economies will benefit from this project by raising the awareness and willingness of investment by stakeholders. Additionally, attendees are encouraged to use the knowledge and information they gain during the conference to influence secondary beneficiaries (such as women in APEC communities who are prone to cervical cancer) in their respective economies. The project will lay the foundation for future actions by public health authorities and healthcare professionals across APEC economies.

Dissemination

To ensure a productive conference, the presentation slides and relevant materials will be collected ahead of the conference and distributed to the participants on site. Documents and materials in the conference, such as conference agenda, feedback evaluation and other materials will be prepared in soft copies, and about 100 copies will be released. In addition, QR codes will be provided to enable public health workers and healthcare professionals who are unable to attend the conference to access relevant information.

The conference report will comply with the APEC rules regarding publications, logo guidelines and copyrights. It will be uploaded to the official website of the Health Promotion Administration (HPA) for general public access.

Linkages

We plan to engage all the ministries/department of health of APEC economies in nominating eligible participants. During the preparation, we will engage the co-sponsoring economies and related agencies. Experts from non- APEC member economies and other international organizations will be invited to participate as either moderators or speakers. In addition, we will also invite experts from APEC and non-APEC member economies, the private sector, academia, NGOs, and other international organizations such as WHO to attend the conference as moderators or speakers. We will remain in close contact with the APEC Secretariat for advice on non-member participation and ensure that the non-member participation guideline are closely followed. To promote cross fora collaboration, we will invite member from APEC Policy Partnership on Women and the Economy(PPWE) and share outputs to participate.

This project will help APEC members economies improve their understanding of cervical cancer prevention and treatment, improve health promotion capabilities, and apply it to the healthcare system and health industry. Therefore, the APEC is considered the most appropriate platform to fund this project with the ultimate goal of strengthening populations' health and sustaining the social-economic development in the region.

Sustainability

This conference will provide a platform for APEC member economies to exchange experiences and share information on HPV vaccination, cervical screening, and medical care for cervical cancer patients. It will help each economy understand the importance of cervical cancer prevention and learn from the successful policies, systems, and experiences of other countries. This will lead to improvements and optimization of existing prevention methods, ultimately enhancing the quality of women's healthcare in the APEC region.

Soft copies of the conference outputs will be disseminated to relevant stakeholders and beneficiaries for future references. Attendees are encouraged to network during the conference to maintain an ongoing exchange of information. We also hope that after the conference, we will promote meaningful communication among various economies on strategies to eliminate cervical cancer. By doing this, we believe that long-term cooperation will be strengthened.

Dr. Wu is currently the Director-General of Health Promotion Administration (HPA), which is the authority responsible for managing the economy-wide health promotion system Chinese Taipei. Dr. Wu envisioned how to accelerate the elimination of cervical cancer and improve women's health. Dr. Wu not only participated in the formulation and promotion of relevant policies, but also promoted them to health units, medical providers and the general public to safeguard women's health.
